How to Prove Income

If your gross annual household income is less than 135% of the federal poverty guidelines, you may qualify for Oregon Lifeline.

To prove your gross annual household income, include any of the following with your application that shows the combined gross income of everyone in your household:

  • The prior year's state or federal tax return
  • Current income statement from an employer
  • Pay stubs for any three consecutive months within the last 12 months
  • Veteran’s administration statement of benefits
  • Unemployment or Workers' Compensation statement of benefits
  • Social Security statement of benefits
  • Retirement or Pension statement of benefits
  • Divorce decree, child support documentation containing income information
